Pasha 24-Hour

Around-the-clock care, when overnight isn't enough.

Two- or three-caregiver rotation so someone is always awake. Used for late-stage dementia, hospice support, and high fall risk.

What's included

The shape of a 24-hour rotation.

Around-the-clock care is a system, not a shift. A small fixed team, a written log at every handoff, and one care plan they all work from.

Every caregiver in the rotation is trained and delegated by our RN for the same nurse-delegated tasks, so medications, insulin, and vitals checks happen correctly at 2pm and 2am. Under a basic license, the medical side of the plan would stop every time the family went to bed.

A two- or three-caregiver rotation, someone always awake
A written log passed at every shift change
Full personal care: bathing, dressing, toileting, transfers
Medication administration around the clock, nurse-delegated
Wandering and fall protocols that hold at 3pm and 3am alike
Coordination with hospice or the medical team when they're involved
When this is right

Three homes where someone is always awake.

Families arrive at 24-hour care when the risk stops keeping business hours.

Late-stage dementia and a door that opens at 2am

Dad started wandering at night and the family was sleeping in shifts. A rotation with someone always awake means the door is watched, the redirection is calm and practiced, and the family sleeps in their own beds again.

Hospice at home, family running on fumes

Hospice manages her comfort, but hospice visits, they don't stay. We hold the hours in between: personal care, repositioning, presence at 4am, and a family that gets to be family for the time that's left.

The third fall was the tie-breaker

Two falls were bad luck. The third meant someone has to be there when he stands up, every time, or the next stop is a facility. Around-the-clock care was the version of safe that let him stay in his own house.

Common questions

What adult children ask first.

No. That would be unsafe for everyone. We build a rotation of two or three caregivers on set shifts, so the person on duty is always rested and always awake. It's the same small team week after week, not a new face every day.
